Abstract:

Objective: The purpose was to identify community characteristics that contribute to reductions in readmission rates and reimbursement penalties for hospital systems in upstate New York. Methods: Hospitals in upstate NY were selected (N = 94). Using an ex post facto design and the ecological model, community characteristics of hospital systems were analyzed and coded. Independent t-tests, ANOVA, and Pearson Correlation tests were conducted. Results: Characteristicscorrelated with reduced hospital readmission rates and reimbursement penalties included hospitals (1) with critical access status; (2) located in counties with a better county health rank; and (3) located in a primary care shortage area that utilized house calls. Discussion: Implications include supporting policies that increase access to services, improve formulas for reimbursement, and encourage innovation in care delivery models. Future research efforts should focus on house calls in primary care shortage areas. Abstract:

Summary Feline sera were submitted to the Cornell Feline Health Center (n = 497) or to the New York State Diagnostic Laboratory (n = 1,565) for feline immunodeficiency virus (fiv) testing. Some sera (n = 166) were submitted for confirmation of previous fiv-positive results; 151 of these sera had been tested at the referring veterinary practice or laboratory, using an in-house elisa. Excluding the samples submitted for confirmation, a total of 173 samples (9.1%) were fiv-positive; 11.6% of the clinically ill or high-risk cats and 0.49% of the healthy, low risk cats were positive for fiv antibody. A commercially available elisa for detection of antibody to fiv was evaluated in relation to the immunofluorescent antibody (ifa) test and the immunoblot assay. The elisa was interpreted according to the manufacturer's instructions, with the ratio of sample optical density to positive control optical density (s/p) determining a positive or negative result. The elisa results based on the s/p interpretation were compared with a kinetics-based (kela) interpretation of the elisa. The kela values were reported as positive, negative, or equivocal. Using the immunoblot as the standard, elisa (s/p interpretation) had sensitivity of 0.93 and specificity of 0.98, whereas the ifa test had sensitivity of 0.95 and specificity of 0.98. However, the sensitivity and specificity of the elisa (s/p interpretation) were markedly reduced for sample results falling in the kela equivocal range, indicating that equivocal results were valid interpretations for some sera. A high number (22.5%) of the samples submitted for confirmation of a positive result from use of the in-house elisa were determined to be negative for fiv antibody. Operator error or incorrect interpretation of the in-house elisa were thought to be the cause of most of these false-positive test results.

Abstract:

Globally, sex workers have highlighted the harms that accompany anti-prostitution efforts advanced via anti-trafficking policy, and there is a growing body of social science research that has emerged documenting how anti-trafficking efforts contribute to carceral and sexual humanitarian interventions. Yet mounting evidence on the harms of anti-trafficking policies has done little to quell the passage of more laws, including policies aimed at stopping sexual exploitation facilitated by technology. The 2018 passage of the Allow States and Victims to Fight Online Sex Trafficking Act (FOSTA) in the U.S. House of Representatives, and the corresponding Senate bill, the Stop Enabling Sex Traffickers Act (SESTA), is a case study in how efforts to curb sexual exploitation online actually heighten vulnerabilities for the people they purport to protect. Drawing on 34 months of ethnographic fieldwork and interviews with sex workers and trafficked persons (n = 58) and key informants (n = 20) in New York and Los Angeles, we analyze FOSTA/SESTA and its harmful effects as a launchpad to more broadly explore how technology, criminalization, shifting governance arrangements, and conservative moralities cohere to exacerbate sex workers’ vulnerability.

Arsitektur vernakular adalah arsitektur yang memiliki respon yang baik terhadap iklim setempat. Hal ini juga berpengaruh terhadap kenyamanan termal dalam bangunan. Sebagai kota yang memiliki iklim tropis lembab, kenyamanan termal bangunan di Kota Pontianak banyak ditentukan oleh pergerakan angin yang terjadi di dalam bangunan. Adaptasi terhadap iklim pada rumah vernakular melayu Pontianak tidak hanya pada penggunaan elemen bangunan seperti bukaan dan bahan bangunan, juga pada tata ruang yang khas, di antaranya terdapat teras, ruang tengah serta pelataran belakang yang memisahkan rumah induk dengan rumah anak. Tujuan penelitian adalah melihat apakah tata ruang ini berpengaruh terhadap penghawaan alami di ruang dalam. Penelitian dilakukan dengan pengukuran di lapangan terhadap variabel kenyamanan termal, terutama temperatur dan kelajuan angin. Kemudian hasil pengukuran dianalisis secara deskriptif kuantitatif dan dilihat hubungan antara variabel dengan tata ruang, yaitu bagaimana temperatur dan kelajuan angin yang berbeda terjadi di setiap ruang, sehingga dapat ditarik kesimpulan. Hasil penelitian menunjukkan bahwa tata ruang di rumah vernakular melayu Pontianak, yaitu hadirnya teras dan pelataran belakang turut berperan dalam penghawaan alami yang terjadi di ruang dalam. Teras berperan dalam menurunkan temperatur luar yang masuk ke dalam bangunan (30,74 °C di ruang luar, lalu 29,84 °C di teras depan, dan 29,09 °C di ruang dalam). Pelataran belakang serta tata ruang dalam memberikan pergerakan angin yang lebih baik, ditunjukkan dengan selisih yang kecil antara kelajuan angin di ruang dalam dengan ruang luar pada rumah dengan pelataran belakang (0,51 m/s) dibandingkan dengan rumah tanpa pelataran belakang (0,77 m/s).Kata-kata Kunci: penghawaan alami, vernakular, tata ruangEFFECT OF SPACE LAYOUT TO NATURAL VENTILATION IN MELAYU PONTIANAK VERNACULAR HOUSEVernacular architecture is architecture that has good response to local climate. This also affects the thermal comfort in the building. As a city that has a humid tropical climate, the thermal comfort of buildings in Kota Pontianak is largely determined by the movement of the wind that occurs inside the building. Adaptation to climate in Pontianak's melayu vernacular house is not only on the use of building elements such as openings and building materials, but also on the typical spatial layout, including a terrace, a central room and a back veranda that separates the main house from the secondary house. The aim of the study was to see whether this spatial arrangement has an effect on natural ventilation in the indoor space. The study was conducted with measurements of thermal comfort variables, especially temperature and wind speed. Then the measurement results were analyzed descriptively quantitatively and viewed the relationship between variables and spatial arrangement – i.e. how the temperatures and wind speed differ in each rooms – so that conclusions could be drawn. The results showed that the layout in Pontianak's melayu vernacular house, namely the presence of terraces and back veranda, played a role in the natural ventilation that occurred in the indoor space. The terrace plays a role in reducing the outside temperature that enters the building (30.74 °C in the outdoor, then 29.84 °C on the front terrace, and 29.09 °C in the indoor). Back veranda and spatial layout provide better wind movement, indicated by small difference between the speed of wind in the indoor and the outdoor space of the house with back veranda (0.51 m/s) compared to the house without back veranda (0.77 m/s).Keywords: natural ventilation, vernacular, spatial layoutREFERENCESBoutet, Terry S. O presente artigo se debruça sobre o conto “Casa Assombrada”, coletado no único volume de contos que Virginia Woolf publicou em vida, Monday or Tuesday (1921), para investigar de que maneira seus contos intensificam a crise dos gêneros literários que seus romances encenam, por um lado; e para entender como tal crise é análoga à questão política que assombra toda sua obra, por outro lado: o gênero enquanto questão identitária. Abstract:

This study aims to increase children English vocabulary aged 5-6 years old by playing CABE or searching and whispering. This research is classroom action research that was conducted in Marsudirini Sang Timur Kindergarten, Salatiga. The Subjects of the study were 20 B1 kindergarten children. Data was collected by teaching English vocabulary by playing CABE in some cycles which includes four stages in the form of cycles, there are (1) planning; (2) implementation; (3) observation; and (4) reflection. Research instruments used in this research was in sheets observation checklist. The results of a percentage of pre-cycle was 13 %, cycle I was 31 % in first meeting and was 66 % in the second meeting, cycle II was 75 % performed in only one meeting. Abstract Although evidence suggests that photographs can enhance persuasive messaging by offering “proof,” less research considers their utility relative to other visual forms that ostensibly convey more information but more abstractly. Drawing on communication and information processing theory, this study examines the influence of visual features and personal experience variables in a domain with urgent need to better understand their role: hurricane messaging. In a between subjects experiment, residents of New York, New Jersey, and Connecticut (N = 1052) were exposed to a hypothetical hurricane forecast accompanied by a photograph of storm surge inundating a house (indexical image), a map of projected storm surge (iconic image), or no image (control), depending on condition. Results revealed that participants in the indexical condition perceived the greatest risk overall and were more likely to mention evacuation as a behavioral intention than did those in the iconic and control conditions, controlling for individual differences (gender, state of residence, etc.). Moreover, risk perception was greatest among residents in the indexical condition reporting fewer personal impacts of hurricanes, suggesting a moderating effect of hurricane experience on risk judgment but not on behavioral intention. Consistent with a dual-process model perspective, when exposed to an image of an identifiable “victim,” participants with less direct experience may have employed an affect heuristic, resulting in heightened risk perceptions. Practically speaking, using evocative photographs as proof may be preferable to a map or text-only approach when warning public audiences of a given hazard, but ethical issues and empirical questions remain.

P.R.57:1 (C18H12CaN2O6S · n H2O, n = 0,1,3) is the most important organic red pigment with a production of more than 50,000 tons per year and an annual sales volume of more than 200 million Euro.[1] In printing ink the pigment is not dissolved, but finely dispersed. Consequently its solid-state properties are maintained. Like most pigments, P.R.57:1 occurs in different crystal phases with different colours. Upon synthesis a trihydrate is formed. Drying at 500C generates a monohydrate with magenta shade, which is used for printing inks. The monohydrate is thermally stable up to temperatures higher than 1900C before it releases water to yield a hygroscopic anhydrous phase with dull dark magenta shade. For all three phases the growth of single crystals is impeded by the low solubility of the pigment in most media. The crystal structures of all three forms were determined from in-house X-ray powder data.[2] The structures were solved by real-space methods with simulated annealing. Subsequently a Rietveld refinement with restraints on bond lengths, bond angles and planar groups was performed. All three phases crystallize in space-group type P21/c, Z = 4. The trihydrate and the monohydrate show eightfold coordination of the Ca ions, the anhydrate a sevenfold one. Apparently the increasing anion-cation interactions lead to the observed colour shift. The arrangement of cations and anions is similar in all three forms. The crystal structures exhibit double layers, one polar, one nonpolar. The polar layer consists of water molecules, calcium ions, sulfonate, keto and carboxylate groups, held together mostly by hydrogen bonds and Coulomb interactions. The nonpolar layer contains naphthalene and toluene moieties.

Abstract: The purpose of this study is to describe the implementation of the One-Roof Junior Secondary School as an alternative to the distribution of nine-year basic education in remote areas in the cultural setting of Central Kalimantan Betang Houses. This research is a qualitative research with a multi-site study design on 3 One-Roof Junior Secondary Schools in Gunung Mas Regency. Data collection is done by methods: in-depth interviews (indepth interview), participant observation (participant observation), and study documentation (study of document). Determination of data sources is done by using purposive sampling technique. Data analysis is done through the activities of organizing data, organizing and dividing data into units that can be managed, mensiteis, looking for patterns, find what is meaningful and what is researched to be decided and reported systematically (Bogdan and Biklen, 1998). Data analysis in this research was carried out in two stages, namely: data analysis for each site (single site) and cross-site data analysis. Checking the validity of the data is done by using a degree of credibility through both source and method triangulation techniques. The results of the study found that the values of the betang house culture that underlies the implementation of the One-Roof Junior Secondary School appeared on: (1) bureaucratic structure, including: SOP, coordination and empowerment of HR; (2) resources, including: human resources, infrastructure and financing; and (3) communication, including: internal communication, with supporting elementary schools, with related agencies, and the community around the school. Penelitian ini merupakan penelitian kualitatif dengan rancangan studi multi situs pada 3 SMPN Satu Atap di wilayah Kabupaten Gunung Mas. Pengumpulan data dilakukan dengan metode: wawancara mendalam (indepth interview), observasi partisipan (participant observation), dan studi dokumentasi (study of document). Penetapan sumber data dilakukan dengan teknik purposive sampling. Analisis data dilakukan melalui kegiatan mengorganisasi data, menata dan membagi data dalam unit-unit yang dapat dikelola, mensitesis, mencari pola, menemukan apa yang bermakna dan apa yang diteliti untuk diputuskan dan dilaporkan dengan sistematis (Bogdan dan Biklen, 1998). Analisis data dalam penelitian ini dilakukan dalam dua tahap, yaitu: analisis data tiap situs (situs tunggal) dan analisis data lintas situs. Pengecekan keabsahan data dilakukan dengan menggunakan derajat kepercayaan (credibility) melalui teknik triangulasi baik sumber maupun metode. Hasil penelitian menemukan bahwa nilai-nilai budaya rumah betang yang mendasari dalam penyelenggaraan SD-SMP Satu Atap muncul pada: (1) struktur birokrasi, meliputi: SOP, koordinasi dan pemberdayaan SDM; (2) sumberdaya, meliputi: SDM, sarana prasarana dan pembiayaan; dan (3) komunikasi, meliputi: komunikasi intern, dengan SD penyangga, dengan dinas terkait, dan masyarakat sekitar sekolah. Em um jogo entre eu-autor e eu-personagem, os diários de escritores muitas vezes encontram-se em um entrelugar na escrita literária. Neste trabalho, enfatizo a construção da ficção dentro do discurso confessional do diário, na transformação da página em branco em laboratório da escrita. Abstract:

This article presents a theoretical review of the main evaluation approaches that marked the trajectory of educational evaluation in the international scenario. This is a bibliographical research that, based on the assessment literature, aimed at studying the various dimensions and different meanings that have historically constituted the educational evaluation field. Seven evaluation approaches are described: goal-based evaluation, evaluation based on scientific logic, value-based evaluation, decision-making evaluation, consumer-oriented evaluation, participant-centered evaluation, and qualitative evaluation. Each one of them is characterized according to its protagonists, objectives, conceptual approaches, theoretical orientations deriving from methodological assumptions, and the scholars who continued their respective approaches. Such international literature had a strong influence on Brazilian evaluation researchers who, based on these references, sought to support a field of knowledge that is still in the process of constitution and strengthening in our country.ResumoEste artigo apresenta uma revisão teórica das principais abordagens de avaliação que marcaram a trajetória da avaliação educacional no cenário internacional. Trata-se de uma pesquisa bibliográfica que, referenciada na literatura da avaliação, busca estudar as variadas dimensões e os diferentes sentidos que constituíram historicamente o campo da avaliação educacional. São descritas sete abordagens de avaliação: a avaliação baseada em objetivos, a avaliação baseada na lógica científica, a avaliação baseada no valor agregado, a avaliação a serviço da decisão, a avaliação orientada para consumidores, a avaliação centrada nos participantes e a avaliação qualitativa. Cada uma dessas abordagens é caracterizada segundo seus protagonistas, objetivos, enfoques conceituais, orientações teóricas decorrentes de pressupostos metodológicos e os estudiosos que deram continuidade nas suas respectivas abordagens. Tatanan spasial (ruang) memperlihatkan hubungan antara arsitektur dan budaya masyarakat setempat. Manusia sebagai makhluk yang berpikir dinamis, memiliki peran besar untuk merubah lingkungan fisik maupun kebudayaan. Tatanan ruang tradisional merupakan warisan leluhur yang harmonis, senantiasa mengalami perubahan untuk beradaptasi dengan modernitas budaya global. Penelitian ini bertujuan untuk (1) mengidentifikasi dan menganalisis unsur-unsur ruang yang berubah dan (2) menjelaskan faktor-faktor sosial-budaya yang mempengaruhinya, pada objek rumah tinggal tradisional di wilayah kebudayaan Jawa Panaragan. Objek penelitian berupa rumah-rumah berlanggam arsitektur Jawa, yang telah berdiri sejak sebelum era kemerdekaan RI, terletak di wilayah tertua dari permukiman Desa Kaponan. Metodologi penelitian menggunakan pendekatan kualitatif-rasionalistik dengan analisis deskriptif. Penggalian data melalui observasi langsung terhadap objek yang menjadi kasus penelitian dan wawancara silang dengan informan (narasumber dan keyperson) terkait. Variabel penelitian meliputi organisasi, fungsi, hirarki, orientasi serta teritori ruang sebagai panduan untuk mengamati perubahan ruang dalam 2 (dua) periode waktu. Objek/kasus penelitian dipilih secara sengaja berdasar kriteria meliputi rumah lurah, carik, pamong desa dan tokoh masyarakat yang menjabat pada masa lampau, dilengkapi dengan rumah petani serta buruh tani. Hasil penelitian menunjukkan bahwa unsur spasial (ruang) yang banyak berubah adalah organisasi dan teritori ruang sebagai konsekuensi dari penambahan jumlah, jenis dan fungsi ruang. Unsur spasial yang sedikit berubah adalah orientasi dan hirarki ruang karena kuatnya faktor kepercayaan leluhur. Faktor yang mempengaruhi terjadinya perubahan ruang terutama adalah struktur keluarga dan perubahan gaya hidup seiring meningkatnya pengetahuan dan pendidikan. Kata-kata kunci : perubahan ruang, rumah tradisional, Jawa Panaragan THE TRADITION BASED ROOM CHANGES IN JAWA PANARAGAN HOUSE OF KAPONAN VILLAGESpatial order (space) shows the relationship between the architecture and the culture of local community. As dynamic thinking creature, human has a major role in changing the physical environment or culture. Order of the traditional spaces which is a harmonious ancestral heritage is constantly changing to adapt to the global culture of modernity. This research aimed to (1) identify and analyze the elements of spatial change and (2) explain the socio-cultural factors that affected it, on the object of traditional house in the Panaragan Javanese cultural area. The object of research were traditional Javanese type of home, built before the era of Indonesia independence (1945), located in the oldest settlement of the Kaponan Village. The research methodology used a qualitative–rationalistic approaches with descriptive analysis. Data mining was conducted through direct observation of objects that became case studies and interviews with related informants and keyperson. Variables of research include organization, function, hierarchy, orientation and territory of spatial (space) as a guide for observing spatial change between two periods of time. Object/case studies were deliberately chosen based on criteria include the house of the village head and officials, teacher and community leaders who served in the past, also added with home of farmers and farmworkers. The results showed that elements of the spatial (space) which was much changed was the organization and territorial spaces as a consequence of the addition of the number, type and function space. The elements of spatial orientation and space hierarchy was less changed, because of the strong ancestral belief and religion. The main factors affecting the occurrence of a spatial change was family structure and lifestyle changes, along with the increasing knowledge and education. Abstract:

Новое антропоморфное изваяние с единичной сарматской тамгой, происходящее из ущелья реки Солонец и расположенное близ сёл Рошиетичий Векь и Рогожены в Северной Молдавии, не относится к типичным сарматским каменным стелам. На личине памятника находится единичный сарматский тамговый знак, имеющий аналогии по схеме начертания в тамговых знаках, встречающихся в Северной Молдавии и Южной Украине. Лицевая сторона антропоморфного изваяния ориентирована на юг и на пологий противоположный склон ущелья, на котором, по всей видимости, находилась древняя дорога, ведущая к броду и водопою. Вместе с тем, стела с единичной сарматской тамгой на личине является центром скопления, состоящего из трёх камней. Камни расположены по разным сторонам от стелы и несут следы механической обработки в виде насечек, выемок, ниш, и желобообразных углублений, что также может говорить о возможной культовой ориентированности комплекса. Антропоморфное изваяние служило указателем места переправы через реку, а также указателем места для забора воды и водопоя лошадей. В тоже время оно могло являться культовым объектом сакрального почитания божества, связанного с культом воды и быть камнем, маркирующим владения сарматов. A different name than English literature, ‘Anglo-Indian Literature’, was given to the body of literature in English that emerged on account of the British interaction with India unlike the case with their interaction with America or Australia or New Zealand. Even the Indians’ contributions (translations as well as creative pieces in English) were classed under the caption ‘Anglo-Indian’ initially but later a different name, ‘Indo-Anglian’, was conceived for the growing variety and volume of writings in English by the Indians. However, unlike the former the latter has not found a favour with the compilers of English dictionaries. With the passage of time the fine line of demarcation drawn on the basis of subject matter and author’s point of view has disappeared and currently even Anglo-Indians’ writings are classed as ‘Indo-Anglian’. Besides contemplating on various connotations of the term ‘Indo-Anglian’ the article discusses the related issues such as: the etymology of the term, fixing the name of its coiner and the date of its first use. In contrast to the opinions of the historians and critics like K R S Iyengar, G P Sarma, M K Naik, Daniela Rogobete, Sachidananda Mohanty, Dilip Chatterjee and Gayatri Chakravorty Spivak it has been brought to light that the term ‘Indo-Anglian’ was first used in 1880 by James Payn to refer to the Indians’ writings in English rather pejoratively. However, Iyengar used it in a positive sense though he himself gave it up soon. The reasons for the wide acceptance of the term, sometimes also for the authors of the sub-continent, by the members of academia all over the world, despite its rejection by Sahitya Akademi (the national body of letters in India), have also been contemplated on. Abstract:

Este estudo descreve e analisa o processo variável da vogal epentética em palavras na língua inglesa iniciadas por clusters por aprendizes brasileiros de inglês como segunda língua (L2). O objetivo dessa pesquisa é, então, identificar a frequência de inserção da vogal de apoio na posição inicial das palavras em língua inglesa que se iniciam com um dos seguintes clusters: /sp/, /st/, /sk/, /sl/, /sm/, e /sn/. O corpus deste estudo é constituído por 18 informantes paraibanos, aprendizes de inglês como L2, estratificados nos níveis básico, intermediário e avançado de proficiência. Os dados mostram que as variáveis sonoridade do encontro consonantal, nível de proficiência, instrução explícita na L2 e contexto precedente foram as mais relevantes à realização do fenômeno. Abstract:

This research was conducted to prove the changes in Malay society by studying the tradition of building houses, in the city of Medan, North Sumatra. The study used a qualitative descriptive method with an ethnomethodology and ethnoscience design, using 10 research subjects on stilt houses. This research instrument uses documentation and interview techniques which comprehensively want to see the cultural phenomenon of the Malay community in the city of Medan, in relation to the tradition of building houses. The results of the data analysis technique used by the Miles and Huberman model show that the change in the shape of the traditional house is caused by the Malay traditional factor which provides openness and flexibility. The changes in the Malay community in the city of Medan have caused the elements of traditional culture to be lost, but it is better that there are new cultural aspects that arise. The phenomenon of cultural transformation of the Malay community so far has taken place according to a flexible framework of customary rules, in responding to changing times it is still acceptable. The implication of the research results is that in some cases there is a tendency that the houses referred to are types of houses on stilts typology I and II, as “traditional identities.” As for residential buildings, “modern identity” is a typology or style III stilt house, which is more varied, monumental and formal. Furthermore, in some buildings, it can be seen that traditional building structures are not integrated with modern structures. Impressed with the value of decorative or ornamental carvings tend to change a lot, especially those of sacred, mystical and religious value. In essence, the Malay tradition of traditional Malay formulates that there are: "three symbols" depicted in the house, namely: the status contained in the hierarchy of people's lives, position, space, and prohibition rules (tabu). Keywords: change, malay society, tradition.AbstrakPenelitian ini dilakukan untuk membuktikan perubahan masyarakat Melayu suatu kajian tradisi membangun rumah tinggal, di kota Medan Sumatera Utara. Penelitian memakai metode deskriptif kualitatif dengan desain etnometodologi dan etnosains, menggunakan subjek penelitian sebanyak 10 buah rumah panggung. Instrumen penelitian ini memakai teknik dokumentasi dan interview yang secara komprehensif ingin melihat fenomena budaya masyarakat Melayu di kota Medan, dalam kaitannya dengan tradisi membangun rumah tinggal. Hasil dari teknik analisis data yang digunakan model Miles dan Huberman menunjukkan bahwa perubahan bentuk rumah tinggal tradisional disebabkan oleh faktor adat Melayu yang memberikan keterbukaan dan keluwesan. Perubahan-perubahan masyarakat Melayu di kota Medan menyebabkan unsur-unsur budaya tradisi ada yang hilang, namun sebaiknya ada aspek budaya baru yang timbul. Fenomena transformasi budaya masyarakat Melayu selama ini berlangsung sesuai kerangka aturan adat yang fleksibel, dalam menyikapi perubahan zaman masih bisa diterima. Implikasi dari hasil penelitian dalam beberapa kasus terlihat ada kecenderungan bahwa rumah tinggal yang dirujuk adalah jenis rumah panggung tipologi I dan II, sebagai “identitas tradisional.” Sedangkan untuk bangunan rumah tinggal “identitas modern” adalah rumah panggung tipologi atau langgam III, ini lebih variatif, monumental dan formal. Selanjutnya pada beberapa bangunan terlihat struktur bangunan tradisi tidak menyatu dengan bangunan struktur modern. Terkesan nilai ragam hias atau ukiran ornamen cenderung banyak perubahan terutama yang bernilai sakral, mistis dan religius. Background: Utilizing a 1-year chart review as the data, Furo et al. conducted a research study on an association between buprenorphine dose and the urine “norbuprenorphine” to “creatinine” ratio and found significant differences in the ratio among 8-, 12-, and 16-mg/day groups with an analysis of variance (ANOVA) test. This study expands the data for a 2-year chart review and is intended to delineate an association between buprenorphine dose and the urine “norbuprenorphine” to “creatinine” ratio with a higher statistical power. Methods: This study performed a 2-year chart review of data for the patients living in a halfway house setting, where their drug administration was closely monitored. The patients were on buprenorphine prescribed at an outpatient clinic for opioid use disorder (OUD), and their buprenorphine prescription and dispensing information were confirmed by the New York Prescription Drug Monitoring Program (PDMP). Urine test results in the electronic health record (EHR) were reviewed, focusing on the “buprenorphine,” “norbuprenorphine,” and “creatinine” levels. The Kruskal–Wallis H and Mann–Whitney U tests were performed to examine an association between buprenorphine dose and the “norbuprenorphine” to “creatinine” ratio. Results: This study included 371 urine samples from 61 consecutive patients and analyzed the data in a manner similar to that described in the study by Furo et al. This study had similar findings with the following exceptions: (1) a mean buprenorphine dose of 11.0 ± 3.8 mg/day with a range of 2 to 20 mg/day; (2) exclusion of 6 urine samples with “creatinine” level <20 mg/dL; (3) minimum “norbuprenorphine” to “creatinine” ratios in the 8-, 12-, and 16-mg/day groups of 0.44 × 10−4 (n = 68), 0.1 × 10−4 (n = 133), and 1.37 × 10−4 (n = 82), respectively; however, after removing the 2 lowest outliers, the minimum “norbuprenorphine” to “creatinine” ratio in the 12-mg/day group was 1.6 × 10−4, similar to the findings in the previous study; and (4) a significant association between buprenorphine dose and the urine “norbuprenorphine” to “creatinine” ratios from the Kruskal-Wallis test ( P < .01). In addition, the median “norbuprenorphine” to “creatinine” ratio had a strong association with buprenorphine dose, and this association could be formulated as: [y = 2.266 ln( x) + 0.8211]. In other words, the median ratios in 8-, 12-, and 16-mg/day groups were 5.53 × 10−4, 6.45 × 10−4, and 7.10 × 10−4, respectively. Therefore, any of the following features should alert providers to further investigate patient treatment compliance: (1) inappropriate substance(s) in urine sample; (2) “creatinine” level <20 mg/dL; (3) “buprenorphine” to “norbuprenorphine” ratio >50:1; (4) buprenorphine dose >24 mg/day; or (5) “norbuprenorphine” to “creatinine” ratios <0.5 × 10−4 in patients who are on 8 mg/day or <1.5 × 10−4 in patients who are on 12 mg/day or more. Conclusion: The results of the present study confirmed those of the previous study regarding an association between buprenorphine dose and the “norbuprenorphine” to “creatinine” ratio, using an expanded data set. Additionally, this study delineated a clearer relationship, focusing on the median “norbuprenorphine” to “creatinine” ratios in different buprenorphine dose groups. These results could help providers interpret urine test results more accurately and apply them to outpatient opioid treatment programs for optimal treatment outcomes.

BackgroundHome oxygen therapy (HOT) is commonly used for patients with severe chronic heart failure (CHF) who have intractable breathlessness. There is no trial evidence to support its use.ObjectivesTo detect whether or not there was a quality-of-life benefit from HOT given as long-term oxygen therapy (LTOT) for at least 15 hours per day in the home, including overnight hours, compared with best medical therapy (BMT) in patients with severely symptomatic CHF.DesignA pragmatic, two-arm, randomised controlled trial recruiting patients with severe CHF. It included a linked qualitative substudy to assess the views of patients using home oxygen, and a free-standing substudy to assess the haemodynamic effects of acute oxygen administration.SettingHeart failure outpatient clinics in hospital or the community, in a range of urban and rural settings.ParticipantsPatients had to have heart failure from any aetiology, New York Heart Association (NYHA) class III/IV symptoms, at least moderate left ventricular systolic dysfunction, and be receiving maximally tolerated medical management. Patients were excluded if they had had a cardiac resynchronisation therapy device implanted within the past 3 months, chronic obstructive pulmonary disease fulfilling the criteria for LTOT or malignant disease that would impair survival or were using a device or medication that would impede their ability to use LTOT.InterventionsPatients received BMT and were randomised (unblinded) to open-label LTOT, prescribed for 15 hours per day including overnight hours, or no oxygen therapy.Main outcome measuresThe primary end point was quality of life as measured by the Minnesota Living with Heart Failure (MLwHF) questionnaire score at 6 months. Secondary outcomes included assessing the effect of LTOT on patient symptoms and disease severity, and assessing its acceptability to patients and carers.ResultsBetween April 2012 and February 2014, 114 patients were randomised to receive either LTOT or BMT. The mean age was 72.3 years [standard deviation (SD) 11.3 years] and 70% were male. Ischaemic heart disease was the cause of heart failure in 84%; 95% were in NYHA class III; the mean left ventricular ejection fraction was 27.8%; and the median N-terminal pro-B-type natriuretic hormone was 2203 ng/l. The primary analysis used a covariance pattern mixed model which included patients only if they provided data for all baseline covariates adjusted for in the model and outcome data for at least one post-randomisation time point (n = 102: intervention,n = 51; control,n = 51). There was no difference in the MLwHF questionnaire score at 6 months between the two arms [at baseline the mean score was 54.0 (SD 18.4) for LTOT and 54.0 (SD 17.9) for BMT; at 6 months the mean score was 48.1 (SD 18.5) for LTOT and 49.0 (SD 20.2) for BMT; adjusted mean difference –0.10, 95% confidence interval (CI) –6.88 to 6.69;p = 0.98]. At 3 months, the adjusted mean MLwHF questionnaire score was lower in the LTOT group (–5.47, 95% CI –10.54 to –0.41;p = 0.03) and breathlessness scores improved, although the effect did not persist to 6 months. There was no effect of LTOT on any secondary measure. There was a greater number of deaths in the BMT arm (n = 12 vs.n = 6). Adherence was poor, with only 11% of patients reporting using the oxygen as prescribed.ConclusionsAlthough the study was significantly underpowered, HOT prescribed for 15 hours per day and subsequently used for a mean of 5.4 hours per day has no impact on quality of life as measured by the MLwHF questionnaire score at 6 months. Suggestions for future research include (1) a trial of patients with severe heart failure randomised to have emergency oxygen supply in the house, supplied by cylinders rather than an oxygen concentrator, powered to detect a reduction in admissions to hospital, and (2) a study of bed-bound patients with heart failure who are in the last few weeks of life, powered to detect changes in symptom severity.Trial registrationCurrent Controlled Trials ISRCTN60260702.FundingThis project was funded by the NIHR Health Technology Assessment programme and will be published in full inHealth Technology Assessment; Vol. 19, No. 75. Abstract:

The city of Indramayu is in the province of West Java which was influenced by Dutch and Chinese culture which left historical buildings. Several previous studies that have been carried out in Indramayu City put more emphasis on historical buildings as the main objects and architectural objects that are considered important by researchers. This study aims to map buildings with Chinese cultural influences in the Chinatown of Indramayu City and identify current historical conditions to clarify which Chinese cultural influences are. This research method is a qualitative description with an analysis of the cultural studies approach. Observation, interviews and documentation are data collection techniques in this study. The results of this study found a typology of mapping of historic buildings in the Chinatown area of Indramayu which can be described and searched for common threads. From the results of the data collection, it can be seen that the elements of Chinese architecture that can be recognized from these buildings are very thick, so that they are clearly part of the historical evidence of the development of Indramayu City as an important city in sea trade. This research is expected to be the basis for producing a document that is used as a guide for further researchers. Keywords: analysis, historic buildings, indramayu, tionghoa. Penelitian ini bertujuan melakukan pemetaan bangunan dengan pengaruh budaya Tionghoa di Pecinan Kota Indramayu dan mengidentifikasi kondisi bersejarah saat ini untuk mengklarifikasi pengaruh budaya Tionghoa mana saja. Metode penelitian ini adalah deskripsi kualitatif dengan analisa pendekatan kajian budaya. Observasi, wawancara dan pendokumentasian merupakan Teknik pengumpulan data pada penelitian ini. Hasil penelitian ini ditemukan tipologi pemetaan bangunan bersejarah di Kawasan pecinan Indramayu yang dapat diuraikan dan dicari benang merahnya. Dari hasil pendataan dapat dilihat bahwa elemen-elemen arsitektur Tionghoa yang dapat dikenali dari bangunan-bangunan tersebut sangat kental, sehingga secara jelas menjadi bagian dari bukti sejarah perkembangan Kota Indramayu adalah kota yang penting di dalam perdagangan laut. Abstract:

BackgroundAtrial fibrillation (AF) can be treated using a maze procedure during planned cardiac surgery, but the effect on clinical patient outcomes, and the cost-effectiveness compared with surgery alone, are uncertain.ObjectivesTo determine whether or not the maze procedure is safe, improves clinical and patient outcomes and is cost-effective for the NHS in patients with AF.DesignMulticentre, Phase III, pragmatic, double-blind, parallel-arm randomised controlled trial. Patients were randomised on a 1 : 1 basis using random permuted blocks, stratified for surgeon and planned procedure.SettingEleven acute NHS specialist cardiac surgical centres.ParticipantsPatients aged ≥ 18 years, scheduled for elective or in-house urgent cardiac surgery, with a documented history (> 3 months) of AF.InterventionsRoutine cardiac surgery with or without an adjunct maze procedure administered by an AF ablation device.Main outcome measuresThe primary outcomes were return to sinus rhythm (SR) at 12 months and quality-adjusted life-years (QALYs) over 2 years after randomisation. Secondary outcomes included return to SR at 2 years, overall and stroke-free survival, drug use, quality of life (QoL), cost-effectiveness and safety.ResultsBetween 25 February 2009 and 6 March 2014, 352 patients were randomised to the control (n = 176) or experimental (n = 176) arms. The odds ratio (OR) for return to SR at 12 months was 2.06 [95% confidence interval (CI) 1.20 to 3.54;p = 0.0091]. The mean difference (95% CI) in QALYs at 2 years between the two trial arms (maze/control) was –0.025 (95% CI 0.129 to 0.078;p = 0.6319). The OR for SR at 2 years was 3.24 (95% CI 1.76 to 5.96). The number of patients requiring anticoagulant drug use was significantly lower in the maze arm from 6 months after the procedure. There were no significant differences between the two arms in operative or overall survival, stroke-free survival, need for cardioversion or permanent pacemaker implants, New York Heart Association Functional Classification (for heart failure), EuroQol-5 Dimensions, three-level version score and Short Form questionnaire-36 items score at any time point. Sixty per cent of patients in each trial arm had a serious adverse event (p = 1.000); most events were mild, but 71 patients (42.5%) in the maze arm and 84 patients (45.5%) in the control arm had moderately severe events; 31 patients (18.6%) in the maze arm and 38 patients (20.5%) in the control arm had severe events. The mean additional cost of the maze procedure was £3533 (95% CI £1321 to £5746); the mean difference in QALYs was –0.022 (95% CI –0.1231 to 0.0791). The maze procedure was not cost-effective at £30,000 per QALY over 2 years in any analysis. In a small substudy, the active left atrial ejection fraction was smaller than that of the control patients (mean difference of –8.03, 95% CI –12.43 to –3.62), but within the predefined clinically equivalent range.LimitationsLow recruitment, early release of trial summaries and intermittent resource-use collection may have introduced bias and imprecise estimates.ConclusionsAblation can be practised safely in routine NHS cardiac surgical settings and increases return to SR rates, but not survival or QoL up to 2 years after surgery. Lower anticoagulant drug use and recovery of left atrial function support anticoagulant drug withdrawal provided that good atrial function is confirmed.Further workContinued follow-up and long-term clinical effectiveness and cost-effectiveness analysis. Abstract:

The text analyzes the changes and crises in the Brazilian political system and its consequences in civil society and scientific production with the victory of Bolsonaro in the 2018 elections. Based on the discussion of historical and recent elements, this article seeks to understand and debate the reasons and transformations in Brazilian society that formed the electoral intent that characterized Bolsonarism. It is shown that this movement is also responsible for one of the most dramatic periods in the country's political history. It ends by analyzing the rupture of Brazilian political stability, attacks on institutions, economic crisis and politicization of science under ideological attack through the pendulum narrowing of Bolsonarism with fascism.ResumoO texto analisa as mudanças e crises no sistema político brasileiro e suas consequências na sociedade civil e produção científica com a vitória de Bolsonaro nas eleições [presidenciais] de 2018. A partir da discussão de elementos históricos e recentes, este artigo busca compreender e debater as razões e transformações na sociedade brasileira que formaram o intento eleitoral que caracterizaram o Bolsonarismo. Abstract:

Knowledge about clean and healthy life in children needs to be given early to shape behavior in everyday life. Knowledge about healthy living can be provided at school through various learning programs. This study aims to find the effectiveness of health promotion programs (JUMSIH) to increase children's knowledge about clean and healthy living. The research method is a pre-experimental one-shot case study design. The respondents of this study were 68 students aged 7-8 years. The results showed that the JUMSIH program can help children have knowledge about healthy living. Based on data analysis, n = 15 generally obtained scores above 2.6. It was concluded that healthy living skills are often performed by students such as hand washing, bathing, and toothbrush behavior which are basic skills for children to be able to live healthy lives. Suggestions for further research which is the development of various programs to increase awareness of clean and healthy living from an early age. Abstract:

RÉSUMÉLe principal objectif de cet article est d’encourager une approche plus large, supraconfessionnelle, du mariage et de la famille à l’époque moderne. La conjugalité a été “désacralisée” par les réformateurs protestants du 16e siècle. Martin Luther, parmi d’autres, a refusé le statut de sacrement au mariage, tout en valorisant celui-ci comme une arme contre le péché. En réaction, le concile de Trente a réaffirmé avec force que le mariage est bien un des sept sacrements chrétiens. Mais, promouvant la supériorité du célibat, l’Église catholique n’a jamais beaucoup insisté sur les vertus de la vie et de la piété familiales avant le 19e siècle. En parallèle, les historiens décèlent des signes de “sacralisation” de la famille protestante à partir du 16e siècle. Leurs conclusions doivent être relativisées à la lumière de recherches plus récentes et plus critiques, centrées sur les rapports et les représentations de genre. Elles peuvent néanmoins inspirer une étude élargie et comparative, inexistante dans l’historiographie traditionnelle, des réalités et des perceptions de la famille chrétienne au-delà des frontières confessionnelles.MOTS-CLÉ: Époque Moderne, mariage, famille, protestantisme, Concile de TrenteABSTRACTThe main purpose of this paper is to encourage a broader supra-confessional approach to the history of marriage and the family in the Early Modern era. Wedlock was “desacralized” by the Protestant reformers of the 16th century. Martin Luther, among others, denied the sacramental status of marriage but valued it as a weapon against sin. In reaction, the Council of Trent reinforced marriage as one of the seven sacraments. But the Catholic Church, which promoted the superiority of celibacy, did little to defend the virtues of family life and piety before the 19th century. In parallel, historians have identified signs of a “sacralization” of the Protestant family since the 16th century. These findings must be relativized in the light of newer and more critical studies on gender relations and representations. Abstract:

Parents play an important role in the development of their children. This research reflects the role of parents in developing children. Through four stages of identification, screening, eligibility, and acceptable results, this method uses a systematic literature review using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) method. The findings from the fourteen articles examined show that parenting skills play an important role in a child's growth and development from birth to death. The determining factor in the development of physical, motoric, moral, language, social-emotional, and life skills aspects is the role of both parents as important teachers for children from birth to adulthood. Parents can also use a variety of parenting strategies and skills, many of which they have learned throughout their lives and passed on to their children, to help their children grow.
